Zane Middle School Excused Absence Form

Use this form to excuse your student from a full day of school (changes cannot be made to attendance after 3 days). Make sure to specify the reason so we can input the correct information. Email address is required and will be checked against student's demographic information. If email address does not match what we have in PowerSchool, the attendance will not be adjusted and you will need to call to update your email address with us.

Loss of Privilege (LOP)/No-Go List

The core values of Eureka City Schools are “safe, respectful & responsible.”  These values are infused into every aspect of school life. If students are repeatedly not meeting the expectations associated with these values, they may lose some school privileges.  Key focus areas are attendance, grades, and behavior.  Students who are in good standing in these three areas have the privilege of access to all events and opportunities associated with the school, including dances, athletics, athletic events and participating in promotion. Students on LOP may be restricted from participating in one or more of the aforementioned opportunities for one quarter of the school year.

Students will be placed on LOP for having the following during a school quarter.

  • 10 or more unexcused tardies 
  • 10% or more absences
  • GPA below 2.0 and no more than 1 F
  • 3 or more major BTFs
  • Suspensions (in-school or out-of school)

If a  student loses privileges due to tardiness or poor attendance, they may be taken off of LOP by attending Saturday Workshop to make up for lost academic time.  1 day of Saturday Workshop for every 10 tardies, 1 day of Saturday Workshop for every day of absence.

If a student loses privileges due to grades, they may be taken off of LOP if they raise their grades above a 2.0 GPA or have fewer than 1 F’s for at least 3 consecutive weeks. The students' grades for the previous quarter will be used to determine standing on the LOP list except for the first and fourth quarters. Grades for 1st quarter LOP will be determined Oct. 7th and May 26th for fourth quarter LOP.  

In order to participate in the end of the quarter Reward Day, students may not have ANY F’s. 

If a student loses privileges due to poor behavior, they may be taken off of LOP by co-creating a Behavior Contract with a school administrator and then completing all of the conditions therein with the exception of end of the quarter reward day, which if lost may not be earned back.  

If students lose athletic privileges they will be held from playing in games until the conditions of LOP are met. 

Parents will be contacted when a student loses their privileges and options for how to return to full privileges will be discussed with a school administrator.
